Registration Instructions: Please fill out the form below to become an affiliate of the Iowa PGA Golf Pass. We will not be paying affiliates by email address so please input your mailing address as well. The Iowa PGA Golf Pass Committee determines eligibility for all Affiliates.
IMPORTANT: Please write down the username and password that you are using to register. After you are registered and you receive your approval email, you will be able to use the login form at the bottom of this page.
